Im unable to make a unban appeal

Ive been banned and im trying to make a unban appeal but it doesn't come up with start new topic what do I do?

So many people are able to daily, this is a case of you not reading / not thinking

To make things a little easier - http://www.altislife.co.uk/forms/5-unban-appeal/ but then again you have already been unbanned so why are you opening an appeal... i am sure we are soon to find out.

same with a game ban
